Complete this form and give it to your employer to nominate REST as your chosen fund. Employers can also give this form to new employees to meet their choice of fund employer obligations.
You can change address or contact details online in MemberAccess. To advise us of a change of name, you need to complete the paper form and provide proof of identity.
The easiest way to notify your TFN is online in MemberAccess.
Complete this form if you wish to change your investment and payment option. Please note your switch will become effective 2 business days after REST receives your request. More info >
To indicate who you want to receive your benefit if you die, you can make or change your non-binding beneficiary nomination online in MemberAccess. To make or change your binding beneficiary nomination you need to complete the paper form.

Complete this form if you wish to pay by cheque your own voluntary contributions or make super contributions on behalf of your spouse. Please note that the receiving spouse must have a REST or Acumen account. You can also make these payments via BPAY
Super and family law payment instruction form
This form is to be completed by the non-member spouse following the split of the member spouse's superannuation benefit as advised by a court order or superannuation agreement.
