Give this letter to your employer as written evidence that REST is a complying super fund and will accept contributions on your behalf. The compliance letter outlines contribution methods for your employer and contains SPIN and ABN information.

Choose REST as your super fund
Complete this form and give it to your employer to nominate REST as your chosen fund.
To rollover your super into REST, the easiest way is to use the online form. Once you have completed it, print it out and send it to us with proof of your idenity. To rollover into Acumen or REST Personal, download the paper form.
Change your membership details (name, contact details, beneficiaries)
You can change address, contact details or beneficiaries online in MemberAccess. To advise us of a change of name, you need to complete the paper form and provide proof of identity.

Choose REST as your super fund
Complete this form and give it to your employer to nominate REST as your chosen fund.
Change your membership details (name, contact details, beneficiaries)
You can change address, contact details or beneficiaries online in MemberAccess. To advise us of a change of name, you need to complete the paper form and provide proof of identity.
The easiest way to notify your TFN is online in MemberAccess.

Use this form to nominate who you want to receive your benefit if you die, or cancel a nomination, or change your current nomination.

Transfers and payments into your account

Pay contributions via cheque or money order
Complete this form if you wish to make a once-off voluntary contribution by cheque or money order to your account.
Pay spouse contributions into your REST or Acumen account
Complete this form if you wish to make super contributions on behalf of your spouse. Please note that the receiving spouse must have a REST, REST Personal or Acumen account.
Rollover part of your super into REST
Complete this form if you wish to transfer part of your superannuation from a non-REST super fund into either your REST Superannuation or REST Pension account.
Super and family law payment instruction form
This form is to be completed by the non-member spouse following the split of the member spouse's superannuation benefit as advised by a court order or superannuation agreement.
Cancel your direct debit arrangement
Complete this form to cancel your voluntary contributions being made via direct debit.
