Complete this form if you would like to transfer your existing Death and/or Total and Permanent Disablement insurance cover to REST or REST Personal

Application to change your investment option
Complete this form if you wish to change your investment and payment option. You can also change these details immediately by logging in to MemberAccess. Please note that switches will be processed 10 days after REST receives your request.

Request to transfer whole balance of super benefits between funds - REST (transfer in)
Complete this form if you wish to transfer (consolidate) the whole balance of your super benefits from another fund to REST. Complete the form by typing in the input fields, then print, sign and return with your proof of identity documents.

Complete this form to advise us of change of name, contact details and/or beneficiaries. You can also change your contact details or beneficiaries immediately by logging in to MemberAccess.

Tax file number (TFN) notification

Complete this form to notify REST of your TFN. You can also notify REST of your TFN online through MemberAccess.

Voluntary contribution voucher
Complete this form if you wish to make a once-off voluntary contribution by cheque or money order into your REST account.

Voluntary contributions - direct debit request
Complete this form if you wish to make regular after-tax voluntary contributions via direct debit into your REST account.
Valid from: 11 September 2009

Spouse contribution payment (REST or REST Personal)
Complete this form if you wish to make super contributions on behalf of your spouse. Please note that the receiving spouse must have a REST or REST Personal account.

Cancellation of direct debit

Complete this form to cancel your voluntary contributions being made via direct debit.

Voluntary contributions- payroll deduction authority
Complete this form only if you wish to make voluntary contributions via your employer's payroll into your REST account.
